    目的 通过检测慢性心力衰竭(CHF)患者GDF-15水平及心脏超声指标评估左室重构,探讨GDF-15的变化对左室重构程度的影响。方法 回顾性分析2015年6月至2016年6月期间在广东省佛山市顺德区容桂街道新容奇医院心血管内科住院的CHF患者共75例,依据引起心力衰竭的病因分为4个亚组:高血压心脏病组(n=22)、冠心病组(n=28)、瓣膜病组(n=17)和扩张性心肌病组(n=8)。另选取同时期80名健康者作为对照组进行比较。测定血清GDF-15及其他相关实验室检查,M型超声测量左心室舒张末期内径(LVEDD)、左心房内径(LAD)和左室射血分数(LVEF)。采用SPSS 13.0软件进行数据分析。计量资料采用均数±标准差([AKx-D]±s)表示,两组间比较采用t检验;计数资料以百分率表示,两组间比较采用χ2检验;两组指标间的相互关系采用Pearson直线相关分析。结果 CHF组的GDF-15水平显著高于对照组[(1195.48±43.26) vs(345.28±42.25)ng/L;P<0.05],各亚组的GDF-15水平间差异均无统计学意义(P>0.05)。GDF-15水平与LVEDD(r=0.63)和LAD(r=0.68)呈显著正相关(P<0.05),与LVEF(r=-0.47)和总胆固醇(r=-0.36)呈显著负相关(P<0.05)。结论 CHF患者的GDF-15水平显著升高。GDF-15可作为间接评价CHF患者心室重构的指标。

    Abstract:

    Objective To investigate the changes of growth differentiation factor-15 (GDF-15) level and echocardiographic parameters to evaluate the left ventricular remodeling in the patients with chronic heart failure (CHF), and determine the effect of GDF-15 levels on the severity of the remodeling. Methods A total of 75 CHF patients admitted in our department from June 2015 to June 2016 were enrolled in this study, and their clinical data were collected and retrospectively analyzed. According to the causes of heart failure, they were divided into 4 subgroups, that is, hypertensive heart disease subgroup (n=22), coronary heart disease subgroup (n=28), valvular disease subgroup (n=17) and dilated cardiomyopathy subgroup (n=8). At the same time, another 80 healthy individuals were recruited as control group. The serum GDF-15 and other related laboratory indicators were measured. The left ventricular end diastolic diameter (LVEDD), left atrial diameter (LAD) and left ventricular ejection fraction (LVEF) were measured by type M ultrasound examination. SPSS statistics 13.0 was used to analyze the data. The measurement data were expressed as mean±standard deviation ([AKx-D]±s), and the Student’s t test was used for the comparison between the 2 groups. The enumeration data were expressed as percentages, and Chi-square test was used for the inter-group comparison. The correlation between the indicators of the 2 groups was analyzed by Pearson linear correlation analysis. Results The level of GDF-15 was significantly higher in the CHF group than in the control group[(1195.48±43.26) vs (345.28±42.25) ng/L, P<0.05]. But there were no significant differences in GDF-15 level in the 4 subgroups (P>0.05). The GDF-15 level was significantly positively correlated with LVEDD (r=0.63, P<0.05) and LAD (r=0.68, P<0.05), and negatively correlated with LVEF (r=-0.47, P<0.05) and total cholesterol (r=-0.36, P<0.05). Conclusion GDF-15 level is obviously higher in the CHF patients. GDF-15 level may be used as an indirect indicator of ventricular remodeling in the patients.
