The trajectory theory is one of the well-developed theories in the field of chronic disease management, and it has been applied to various extents in both domestic and abroad clinical practice. In this paper, we summarized the main content, development and implementation of the trajectory theory for chronic disease management. In combination with its current development trend, the theory should be further integrated with the culture and reality in China, and applied in the program development of information technology so as to provide new ideas for the development of chronic disease management in the future.
