Prevalence of tension-type headache in informal caregivers of patients with advanced dementia and its influencing factors

    Abstract:

    Objective To investigate the prevalence, clinical features and influencing factors of tension-type headache (TTH) in informal caregivers of patients with advanced dementia. Methods A total of 136 hospitalized patients with advanced dementia diagnosed in our center from January 2017 to January 2019 were consecutively recruited in the study, and among the 108 informal caregivers whom they had, 80 caregivers were randomly sampled by lottery to accept a face-to-face questionnaire for headache. Their demographic data and the related questions about headache diagnosis were obtained through the questionnaire. The caregivers were divided into TTH group (n=29) and the non-TTH group (n=51) according to whether they had TTH or not. SPSS 22.0 software was used for statistical analysis. Multivariate logistic regression analysis was used to screen the independent risk factors. Results The prevalence of TTH was 36.3% (29/80) in the informal caregivers of advanced dementia patients. Bilateral headache (62.1%), non-pulsating pain (75.9%), moderate pain (75.9%) and frequent headache (51.7%) were very common manifestations for the TTH caregivers. Phonophobia (20.7%) was the most common accompanying symptom. Logistic multiple regression analysis showed that care period <5 years (OR=5.66,95%CI 1.62-19.79, P=0.009), constipation (OR=10.70,95%CI 1.65-69.51, P=0.013) and chronic gastritis (OR=5.46,95%CI 1.02-29.10, P=0.047) were the risk factors for TTH in the caregivers. Conclusion The prevalence of TTH is higher in the informal caregivers for patients with advanced demential, and the caregivers with care period <5 years, constipation and chronic gastritis are susceptible to suffer from TTH.
